On my 212 with the Kohler 12 HP (K301AQS) the John Deere part number, M47494, is a paper filter.
My question is why doesn't the OEM filter include a pre-filter like around 90% of all other engines do?

Just a guess.
1. Cost saving to Deere.
2. Foam pre-filters are supposed to be reusable - wash it and lightly re-oil it. Doesn't everyone do this instead of installing a new pre-filter every time the paper filter is replaced?
My bet is that (1) is the reason, and (2) is the Deere company public response if asked.

Guess I should have said that I ordered one with a pre-filter since there wasn't one (pre-filter) on the machine when I got it. I just feel better with one and wanted to make sure using the thing wouldn't screw something up due to air/fuel mixture type things.
A pre filter is preferred, but being washable they have a longer life then the paper ones. Many people are also too cheap to buy both and do it right.

I like the foam pre-filter. I kept one from an old filter so each time I service the machine I swap to a fresh foam pre-filter and wash the old one. The paper inner filter stays clean and I avoid any build up of debris on the outer filter. I swap pre-filters 4 times or so before changing the paper filter.
