I have an x320 2008 350hours with a new problem with the Idler Pulleys getting damaged. In the image below this is pulley 4 and 15 both plastic.

I replaced both pulleys along with the belt, tension spring and belt guides but after mowing for three times the 15 (AM138079) flat pulley is damaged again.

What would be causing this to happen?

The belt seems a little loose but not alot.
Is there another way to tighten the belt(except for the tension spring)? Also I replace with an OEM belt so that should also not be a problem (Part number was M144044).

Does the pulley hook to the second or first hole on the idler bracket?

Looks like blunt force trauma to me. Make sure the pulleys are in the correct position. Not sure which hole it goes in. It should be as close as possible to the belt guide. There is no adjustment, just spring tension.
I have seen these pulleys damaged in shipping. They get dropped and the edge gets cracked. Maybe you didnt notice it when you installed it. Take the part back and see what they say.

I got everything back together today with a brand new flat pulley.

Started up the mower and I seem to have excessive vibration and belt slap from the drive belt again. So much that I think the long side of the belt might be hitting the flat pulley. If I increase the throttle it only gets worse.

I am hearing some grinding from the PTO clutch. Would a bad PTO clutch cause the belt to slap really bad? It does not appear that the belt is loose when not running. Also seems to have normal power when driving.
